Recertification

Training Requirements 

Providers, including key practice staff assuming key program roles, must complete any required training that satisfies educational requirements for program participation.  New training, or a review of previous training, might be required to stay informed as program requirements may change slightly each year. Refer to EZIZ Training for a listing of required training by vaccine program and role.

How to Prepare 

Providers must update their provider profiles during recertification. Gather required information: 

  • Contact information for key practice staff including providers with prescription-writing privileges who administer vaccines. 
  • Medical license numbers and National Provider Identification (NPI) numbers
  • Provider shipping address, delivery days & hours. 
  • Names and contact information for key practice staff. 
  • Brands/models of refrigerators and freezers storing public vaccines. 
  • Type, serial number, and calibration expiration date of data loggers (primary & backup). 
  • Number of patients eligible for private and publicly funded vaccines to be immunized in the coming year based on previous year’s data (VFC, VFA & LHD 317 providers only)

How to Recertify

1.Ensure your account is in good standing. 
Provider accounts that are SUSPENDED due to Mandatory Corrective Actions will not be able to access the myCAvax Recertification form. 
2.Gather required information to update the provider profile. 
3.Complete required EZIZ Training
EZIZ Learning History page identifies lessons providers may test out of if lessons were successfully completed the prior year. To receive credit for the training requirement, key practice staff must login to their unique EZIZ Training account to test out of or complete all required lessons once available.  
4.Login to myCAvax to access the Recertification form. 
5.Verify and update your practice information. 
Carefully review address, registry ID, delivery times, key practice staff, patient estimates, vaccine storage and data logger equipment, and healthcare providers who will be administering immunizations. Gather Certificates of Calibration for primary & backup data loggers. 
6.Submit for Provider of Record’s review and e-signature. 
Provider of Record must review and electronically acknowledge compliance with all items outlined in any provider agreements. An email with a link to DocuSign will be sent to the Provider of Record. Recertification is not complete until agreements have been signed.
