Reporting Shipment Incidents
Providers must report all shipment incidents in myCAvax as soon as discovered for resolution. Timing is critical. McKesson requires that shipment incidents be reported the day of receipt for resolution. Gather the information needed to report the shipment incident. Please make sure data is accurate and complete.
Incident Types
These incident types are listed in myCAvax:
- Previously opened
- Broken, torn, or tampered with
- Not ordered or incorrect recipient
- Package never arrived
- Out-of-range temperatures during transit
- Shortage/overage of vaccines received
