Vaccine Management Plan

About the Template 

Providers complete the vaccine management plan (Word) template, and the Mobile Vaccine Management Plan (Word) if applicable, and add their own practice-specific protocols.  Vaccine Coordinators should study the provider’s plan and be familiar with all routine and emergency tasks. 

Key Practice Staff 

The template defines responsibilities for required and optional program roles. Responsibilities reflect requirements and best practices that protect vaccines. Make sure staff filling these roles understand their roles and responsibilities. 

Routine Situations 

The template includes a quick-start guide of routine tasks staff might perform, including initial equipment setup, twice daily temperature monitoring, monthly vaccine inventories, and annual update of vaccine management plans.  

Emergency Preparedness 

The template helps to structure preparation and response so staff have the confidence to respond to situations that may impact vaccine stability and staff safety. Providers should be actively involved in preparations to ensure staff are properly trained to respond to emergencies. 

This section includes key contact information, location of emergency transport supplies, and facility floor plan. It also includes actions staff will take before, during, and after an emergency and should be guided by provider input. 

A key component of any emergency plan is an alternate vaccine storage location, a pre-arranged, compliant facility that could store vaccines in the event of an emergency. Key requirements:  

  • Compliance: Must have CDPH-compliant vaccine storage units monitored with calibrated digital data loggers  
  • Capacity: Large enough to hold provider’s entire vaccine inventory 
  • Backup Power: Ideally equipped with a backup generator 
  • Dedicated Use: For vaccine storage, not food or beverages
